臺灣COVID-19疫情期間社區老年人社會網絡與衰弱或肌少症之相關研究

Abstract
本研究之目的主要在了解在2021年COVID-19期間臺灣社區老年人的社會網絡、健康狀況、健康行為與衰弱或肌少症之間的情形及對衰弱或肌少症之預測力。本研究採次級資料分析,對象為2021年1到10月期間在臺北市某地區醫院接受老年健康檢查等醫療服務的社區老年人,總共分析981名社區老年人的資料,研究結果如下:1.本研究族群平均年齡約為74.49歲、女性58.1%、有13.3%社區老年人衰弱、14.5%社區老年人罹患肌少症、社會網絡得分平均為15.46。2.雙變項分析結果顯示與衰弱相關因素包括:年齡、社會網絡、腎功能、認知狀態、憂鬱情緒、飲酒及運動。此外,與肌少症有相關因素包括:年齡、社會網絡、代謝症候群、身體質量、認知狀態、飲酒及運動。3.多元羅吉斯迴歸分析結果顯示年齡越大、社會網絡越差、腎功能異常及有憂鬱情緒較易有衰弱。此外,年齡越大、社會網絡越差、身體質量過輕及每周運動不足150分鐘較易罹患肌少症。根據本研究結果發現社區老年人社會網絡越差會影響衰弱與肌少症,建議鼓勵社區老年人積極社會參與對於健康有一定的助益,也能夠讓老年人維持生活品質。
This study aims to examine the relationship between social networks, health status, health behaviors, frailty, and sarcopenia among community-dwelling older adults in Taiwan. Secondary data analysis was conducted using data from elderly health check-ups and medical services in Taipei City from January to October 2021. A total of 981 community-dwelling older adults were included in the analysis. The results were as follows.1.The average age of the participants was approximately 74.49 years, with 58.1% being female. About 13.3% of the community older adults met the criteria of frailty, and 14.5% were diagnosed with sarcopenia. The average social network score was 15.46.2.Bivariate analysis identified several factors associated with frailty, including age, social network, kidney function, cognitive status, depressive emotions,alcohol consumption, and exercise. Factors associated with sarcopenia included age, social network, metabolic syndrome, body mass index, cognitive status, alcohol consumption, and exercise.3.Multiple logistic regression analysis revealed that older age, poorer social network, abnormal kidney function, and depressive emotions were associated with a higher risk of frailty. Similarly, older age, poorer social network, underweight body mass index, and inadequate weekly exercise (<150 minutes) were associated with a higher risk of sarcopenia. Based on these findings, it is recommended to promote active social participation among older adults, as it can positively impact their health and help maintain their quality of life.
