國民中學衛生組業務內容與負擔合理性之探究-以新竹縣市為例

Abstract
本研究旨在探討國民中學衛生組業務內容與負擔合理性,透過文獻探討與公文分析了解業務內容,並透過訪談衛生組長了解業務負擔情況,並提出相關建議,作為未來研究與政策擬定之參考。本研究採質性研究,透過文件分析法與半結構式訪談法,分析統整獲得以下研究結論:一、在公文來源、性質與內容之分析部分(一)公文來源以新竹市政府教育處體育保健科為最多,又以轉發衛福部最多。(二)公文性質以衛生計畫業務為最多,其中以健康促進計畫為主要業務類別。(三)公文內容以衛生保健、健康促進計畫及環境教育三大業務數量為最多;而衛生組實際處理的公文內容以「學校健康促進計畫六大議題」為最多。二、在業務相關性、助益性及助益性之分析部分(一)在相關性面向:
1.業務大部分具有高相關性;
2.相關性低公文數量較少,處理所需時間也較少,非衛生組主要業務負擔;
3.相關性受各校分工與承辦人主觀認定而影響。
(二)在助益性面向:
1.健康促進計畫業務雖有助益性,但僅限於知識層面及部分學生;
2.環境整潔教育及流感疫苗施打對學校及學生有整體長期助益性。
(三)在效能性面向:
1.疫苗接種與整潔教育具有高度效能性;
2.健康促進計畫能達成目標的效能性不高,且未能達成長期習慣的目標。
(四)整體而言,業務的相關性最高、助益性次之,而效能性偏低。
三、在衛生組長對業務內容的評價與建議之部分
(一)對業務內容的評價:
1.整潔是公文看不到的業務量,卻是每日衛生組的重點;
2.健康促進計畫的數據僅能反映部分真實,只看短期,忽略長期;
3.只重政策理想,忽略學校情境;
4.只見片面減量,忽略整體總量。
(二)對業務推動的建議:
1.健康促進計畫將課程納入規劃,並拉長前後測時程;
2.政策要考量各校現況,結合現有日常,避免外加式表面成果。
依據上述研究結論,本研究提出對主管機關及未來研究之建議,作為後續。
The purpose of this research is to explore the job content and reasonableness of the workload of the Hygiene Section in junior high schools. The study aims to understand the job content through literature review and official document analysis, and to assess the workload through interviews with the Section Chief of Hygiene. Additionally, relevant suggestions are provided as references for future research and policy formulation.This research adopts a qualitative approach, using official document analysis and semi-structured interviews, leading to the following conclusions:1. Analysis of the source, nature and content of official documents:(1)The most common source of official documents is the Physical and Health Education Section of the Education Department of the Hsinchu City Government, and the most is forwarded to the Ministry of Health and Welfare.(2)Hygiene planning affairs is the largest item of the official documents. Among them, the health promotion program is the main job category.(3)The content of official documents primarily involves three major areas: hygienic health care, health promotion programs, and environmental education. Six major topics of the school health promotion program rank first among the official documents.2. Analysis of job relevance, helpfulness and effectiveness(1)Relevance: a. Most of the job content is highly relevant. b. The number of official documents with low relevance is relatively small and the processing time of the documents is also less, indicating that the official document processing is not a major work burden of the Hygiene Section. c.The relevance perspective is influenced by the division of labor in each school and the subjective cognition of the responsible person. (2)Helpfulness a. Although the health promotion program is useful, it is limited to the knowledge level and affects only some students.b. Environmental cleanliness education and influenza vaccination offer overall long-term benefits for schools and students.(3)Effectiveness:a. Environmental cleanliness education and influenza vaccination are highly effective.b. The effectiveness of the health promotion program in achieving its goals is not high, and the long-term establishment of habits is also low.(4)Overall, job relevance is the highest, helpfulness being the second, and the effectiveness is the lowest.3. Evaluation and Suggestions from the Section Chief of Hygiene(1)Evaluation of Job Content:a. Environmental Cleanliness is a key job not visible in official documents but is a daily focus of the Hygiene Section.b. Data from the health promotion program reflects only a partial truth, focusing on the short-term while ignoring long-term effects.c. Policies often focus on ideals and overlook school contexts.d. Policies to reduce work load only see partial reductions, neglecting the overall workload of the job.(2)Suggestions for job promotion:a. The health promotion program should incorporate the curriculum into the plan and extend the duration of pre- and post-test periods.b.Policies should consider the current situation of each school, integrat with existing daily activities, and avoid superficial results.Based on these conclusions, this research proposes recommendations for the competent authorities and future studies.
