足弓襪套對功能性扁平足行走與跑步時的步態及足壓的差異

Abstract
背景:扁平足是一種常見的足部病理現象,定義為足弓平坦或消失,而功能性扁平足也很常在有運動習慣的人身上發現。先前研究已證實,扁平足會影響下肢的生物力學參數,例如:步態週期中站立期較長、跑步時足部內旋角度較大,且具有更高的足底內側壓力。先前研究指出有效的矯正方式有貼紮、鞋墊、復健訓練等,但對於足弓襪套的介入效果,目前仍是未知。目的:探討功能性扁平足在有無足弓襪套支撐下行走與跑步時的步態及足壓的差異。方法:招募20名有規律運動的功能性扁平足受試者,分別檢測有無襪套介入的靜態站立、行走與跑步時的足壓,並同時測量走路時的步態週期及跑步時的足部內旋角度,而足壓數據使用Pedar-X® 足壓測量系統測量、步態週期及足部內旋數據則使用Gait Up Physilog®5 步態分析儀收集。以相依樣本t檢定分析足弓襪套介入前後的差異,顯著水準定為α= .05。結果:足弓襪套可以降低受試者站立時的足底內側壓力,並且可以減少行走時步態的站立期時間以及跑步時足部旋前的角度,但是行走與跑步時的足底內側壓力無顯著差異。結論: 透過本研究的結果可以發現足弓襪套能夠有效的降低功能性扁平足受試者跑步時足部落地的旋前角度、行走時的站立期時間,甚至能達到貼紮以及矯正鞋墊的部分效果,例如降低站立時的足底內側壓力,未來功能性扁平足矯正方式的研究、產品設計可朝此方向發展。
Background: Flatfeet is a common type of foot conditions, defined as flat or absent arches, and flexible flatfeet also commonly found in people with exercise habits. Previous studies have pointed out that flatfeet affect lower extremity biomechanical parameters such as: longer stance phase in the gait cycle, greater foot pronation during running, and higher medial plantar pressure. Previous studies also suggested that effective correction methods include taping, insole, rehabilitation training, etc., but the intervention effect of arch socks is still unknown. Purpose: To investigate the differences in gait and foot pressure between walking and running with or without arch sock support for flexible flatfeet. Methods: Recruit 20 flexible flatfeet who exercise regularly, and the foot pressure during static standing, walking and running with or without sock intervention will be measured, and the gait cycle during walking and the pronation angle of the foot during running will be measured at the same time. Foot pressure data will be measured by the Pedar-X® Foot Pressure Measurement System, and gait cycle and foot pronation data will be collected by the Gait Up Physilog® 5 Gait Analyzer. Paired sample t-test will be used to analyze the differences before and after the intervention of the arch socks, and the significance level will be defined at α= .05. Results: The arch socks can reduce the medial plantar pressure during standing, as well as decrease the stance phase in the gait cycle and the foot pronation during running. Conclusion: Based on the Results of this study, it can be concluded that arch socks can effectively reduce the foot pronation during running and the stance phase duration during walking in flexible flatfeet . They can even achieve partial effects similar to taping and foot insoles, such as reducing medial plantar pressure during standing. Future research and product designs for the correction of functional flat feet can consider this approach.
