安非他命緩起訴附命戒癮工作網絡的溝通:一線工作者觀點

Abstract

本研究目的是探討臨床藥癮相關業務網絡以及工作者所面臨的困境。先了解醫療院所從服務工作中給予藥癮者的資源,進而評估能提供藥癮者的效益程度,並分析強化的業務範疇的機會,藉由訪談而討論出的研究結論,擬定出潛在欠缺資源的工作者,以共同連結的網絡系統,提供未來的藥癮者一個完善、效率高、不間斷的連結式賦歸計畫。研究過程中會徵求不同專長人員進行深入訪談,訪問內容以民國113年質性半結構式的兩個主題進行提問:1.「在二級安非他命施用者附命戒癮治療中,執行上面臨的困境?」2.「以自身角度,提供可合作及聯手共同處理業務項目?」。其中,藥癮相關的工作者服務範圍不重複,以醫師、個案管理師、觀護人、護理師、心理師、社工師為訪談對象,與其深入探究工作常面臨處遇計畫中的困難,並探討是否可由其他資源的介入或協助來加以共同解決問題。選定附命緩起訴工作者的緣由是經常觀察到個案接受醫療治療並回地檢署取消起訴後,仍無法順利找到歸屬、人生目標、或銜接合適的機構。在回歸社會的道路上因為迷失方向而再次使用藥物或成為失蹤人口,故期望在進行研究的過程中可以找尋到單位間串聯的業務並擬出一套賦歸可行性高的流程供藥癮者遵循。在研究的結果中進行內容分析整理所有訪談者的資料,並提出建議:針對實務工作者的困境,多數人認為個案容易失聯且追蹤困難,再者是藥癮者無動機或是反覆中斷緩起訴附命戒癮治療,同時一線人員在業務上認為有執行的壓力、資訊互通不順暢等情形,因此藥癮工作者自身的看法是期待有額外的跨團隊合作機會,建議提供所有相關工作者一個更加便利及互助合作的網絡系統,加強各部門的關係連結,並提供個案持續維持醫療及賦歸社會的機會,作者期待給予未來執行毒品相關政策者們一個實際狀況上的參考,並也期望藥癮者將得到更加完善的服務。
The purpose of this study is to explore the clinical service network related to addiction and the challenges faced by professionals working in this field. It first investigates the resources provided by medical institutions for individuals with substance use disorders, evaluates the effectiveness of these resources, and analyzes opportunities to strengthen service domains. Based on findings from in-depth interviews, the study aims to identify professionals lacking adequate resources and propose the establishment of an interconnected support system. This system would serve as a comprehensive, efficient, and continuous reintegration plan for individuals with addiction. The research involves qualitative semi-structured interviews conducted in 2024, targeting professionals from various fields. The interview questions are centered on two main themes:1.”What challenges do you face in implementing court-mandated addiction treatment for individuals using methamphetamine (Category II)?”, 2.“From your perspective, what collaborative or jointly handled tasks could be developed?” Interview participants include physicians, case managers, probation officers, nurses, psychologists, and social workers, each representing distinct roles within the addiction treatment network. The goal is to understand the difficulties encountered in treatment planning and explore whether these issues can be resolved through the involvement of additional resources or inter-agency collaboration. The focus on professionals involved in deferred prosecution cases stems from frequent observations that, even after receiving medical treatment and having charges dismissed by the District Prosecutors Office, many individuals struggle to find a sense of belonging, life direction, or suitable institutional support. Without proper guidance, these individuals often relapse or go missing after returning to society. This study thus aims to identify the connections between relevant agencies and propose a highly feasible reintegration process for future clients to follow. The data collected from interviews are analyzed using content analysis. The findings indicate several key challenges reported by frontline workers: clients are often difficult to track or lose contact, and many lack motivation or frequently interrupt their court-mandated treatment process. In addition, professionals report high work pressure and inadequate information flow across institutions. Consequently, there is a strong desire among addiction-related professionals for enhanced opportunities for cross-team collaboration. The study recommends the development of a more convenient and cooperative network system for all relevant professionals, in order to strengthen interdepartmental connections and provide clients with continued medical support and pathways to reintegration. The author hopes this research can serve as a practical reference for policymakers working on drug-related interventions, and ultimately contribute to a more comprehensive and effective service system for individuals with addiction.
